Retatrutide is an investigational synthetic peptide that acts on three receptor targets at once: glucose-dependent insulinotropic polypeptide, glucagon-like peptide-1, and glucagon. It is developed by Eli Lilly and appears in the literature and in trial registries under the code LY3437943. The molecule belongs to a class of engineered peptides designed to resist rapid breakdown and permit infrequent subcutaneous administration. No regulatory agency has approved it for clinical use, and all available human data come from controlled trials rather than from routine practice.
The intended pharmacology combines three signals in one molecule. GLP-1 receptor activation reduces appetite and slows gastric emptying, effects already exploited by approved incretin-based therapies. GIP receptor engagement is associated with improved insulin sensitivity and with direct effects on adipose tissue, although how much it contributes to overall outcomes is still debated. Glucagon receptor agonism raises energy expenditure and supports hepatic lipid handling, a mechanism that also tends to increase glucose output. The triple profile is hypothesized to produce a larger metabolic effect than single or dual agonism, but the relative weight of each receptor in humans is not settled.
Human evidence remains limited to controlled studies. A phase 2 trial in adults with obesity reported large, dose-dependent reductions in body weight over 48 weeks, with gastrointestinal events as the most frequently recorded adverse effect. Phase 3 programs designated TRIUMPH, for obesity, and TRANSCEND, for type 2 diabetes, are intended to confirm efficacy and to characterize safety in larger populations. Related studies are examining conditions such as knee osteoarthritis in people with obesity and metabolic liver disease. Open questions include long-term tolerability, effects on lean mass, and what happens after treatment is stopped.

The three-receptor design places retatrutide in a distinct category relative to older incretin-based therapies. Single agonists target one receptor, and dual agonists target two. Adding a third target broadens the pharmacological footprint and introduces new trade-offs among efficacy, tolerability, and glucose control. How these trade-offs resolve in large trials is a central focus of current research.
Retatrutide is an investigational peptide studied for obesity and type 2 diabetes. It is a single synthetic molecule designed to activate three metabolic receptors simultaneously. The compound belongs to the incretin mimetic family, a group of peptides that imitate gut hormones involved in appetite and glucose control. Its research code is LY3437943, and it remains under clinical study rather than cleared for routine medical use.
Receptor activation produces downstream effects that differ by tissue. GLP-1 receptor signaling influences appetite regulation and insulin secretion in a glucose-dependent manner. GIP receptor activity contributes to metabolic handling of nutrients and may modulate adipose tissue. Glucagon receptor engagement raises energy expenditure and promotes hepatic lipid turnover, though the balance among these actions in humans is still being characterized. Preclinical models showed reductions in body weight and improved glycemic markers.
Clinical development has progressed through phase 2 trials in adults with obesity and type 2 diabetes, with phase 3 programs reported as ongoing. Reported outcomes include reductions in body weight and improvements in glycemic measures over defined treatment periods. Whether these effects translate into durable benefits after treatment ends remains an open question. Long-term safety data across broad populations are not yet complete, and regulatory decisions have not been announced.
Studies in the hematopoietic system disclosed that during endothelial to hematopoietic stem cell transition, ADGRG1 is a transcriptional target of the heptad complex of hematopoietic transcription factors, and is required for hematopoietic cluster formation. Recently, two studies showed that ADGRG1, is a cell autonomous regulator of oligodendrocyte development through Gα12/13 proteins and Rho activation. Della Chiesa et al. demonstrate that ADGRG1 is expressed on CD56dull natural killer (NK) cells. Lin and Hamann's group show all human cytotoxic lymphocytes, including CD56dull NK cells and CD27–CD45RA+ effector-type CD8+ T cells, express ADGRG1.
==== Mechanism of action ==== Propranolol is a non-selective beta receptor antagonist. This means that it does not have preference to β1 or β2 receptors. It competes with sympathomimetic neurotransmitters for binding to receptors, which inhibits sympathetic stimulation of the heart. Blockage of neurotransmitter binding to β1 receptors on cardiac myocytes inhibits activation of adenylate cyclase, which in turn inhibits cAMP synthesis leading to reduced Protein kinase A (PKA) activation. This results in less calcium influx to cardiac myocytes through voltage-gated L-type calcium channels, meaning there is a decreased sympathetic effect on cardiac cells, resulting in antihypertensive effects including reduced heart rate and lower arterial blood pressure. Blocking neurotransmitter activity at β2 receptors in vascular smooth muscle cells causes vasoconstriction, leading to hypertension.
=== Continuous and individualized delivery systems === As Parkinson's disease advances, the duration, efficacy and reliability of levodopa progressively decline producing unstable plasma concentration and motor fluctuations. The fluctuations contribute to a series of motor and non-motor complications including wearing off and levodopa induced dyskinesia that need a better tailored pharmaceutical approach to the disease. A steady activation of the dopaminergic receptors was initially pursued through intravenous infusion which shortly was replaced by a series of new levodopa device-aided formulations. Duodopa or Levodopa-carbidopa intestinal gel (LCIG) is a methylcellulose-based suspension of levodopa and carbidopa delivered in a continuous way into the duodenum or jejunum through a portable pump and a PEG-J tube system. In randomized studies, LCIG proved to reduce daily OFF time and increase ON time without troublesome dyskinesia but, at the same time, carries with it problems related to the infusion system, such as PEG-J and stoma complications. Produodopa is a soluble prodrug formulation of fosfolevodopa/foscarbidopa developed for a 24 hour subcutaneous infusion. After the infusion, the prodrug is converted by phosphatases into active levodopa and carbidopa that manage to stabilize their concentration in the blood. Compared with levodopa/carbidopa, the prodrug approach increases ON time without troublesome dyskinesia by 1.75 hours and reduced OFF time by 1.79 hours other than improvements also in quality of life, motor fluctuations and sleep.

VIP is highly localised in lungs (70%) and binds with alveolar type II (AT II) cells via VPAC1. The biological (vasodilator) activity of vasoactive intestinal peptide (VIP) was discovered in the lungs before the peptide was isolated and chemical identity characterized from intestine. VIP levels are also considerably high in the brain and the gut. It is localized in key sites in the lung, has potent activities on its major functions, and appears to play an important role in pulmonary physiology and disease. The principal localization of VIP-containing neurons in the tracheobronchial tree is in the smooth muscle layer, around submucosal mucous glands and in the walls of pulmonary and bronchial arteries. Immunoreactive VIP is also present in neuronal cell bodies forming microglia that provide a source of intrinsic innervation of pulmonary structures.
=== Organoberkelium compounds === Berkelium forms a trigonal (η5–C5H5)3Bk metallocene complex with three cyclopentadienyl rings, which can be synthesized by reacting berkelium(III) chloride with the molten beryllocene (Be(C5H5)2) at about 70 °C. It has an amber color and a density of 2.47 g/cm3. The complex is stable to heating to at least 250 °C, and sublimates without melting at about 350 °C. The high radioactivity of berkelium gradually destroys the compound (within a period of weeks). One cyclopentadienyl ring in (η5–C5H5)3Bk can be substituted by chlorine to yield [Bk(C5H5)2Cl]2. The optical absorption spectra of this compound are very similar to those of (η5–C5H5)3Bk. Berkelium also forms berkelocene, an actinocene complex, with substituted cyclooctatetraenides.

It is an investigational peptide that activates three hormone receptors: GIP, GLP-1 and glucagon. It is being studied mainly for obesity and type 2 diabetes, and it is not approved for any clinical use. Published information comes from controlled trials rather than from general practice.
No. As of the most recent public information it remains investigational in every jurisdiction. Material sold under this name outside trials is a research chemical, not an approved medicine. Current status should always be checked against regulator notices.
Dual agonists act on two receptors, usually GIP and GLP-1. Retatrutide adds glucagon receptor activity, which is associated with increased energy expenditure. Whether that third component adds clinically meaningful benefit over dual agonism remains an open question.
